Chronic Care Program

The Bonnyville/Aspen Primary Care Network provides co-ordinated health care for people with chronic diseases in the community of Bonnyville and the surrounding areas.

The Primary Care Network has 4 nurses to assist the physicians in seeing patients with multiple medical problems such as high blood pressure, high cholesterol, diabetes, chronic bronchitis, rheumatoid arthritis, and other long-term health problems that need to see their doctor on a more regular basis.

The nurses assist the physicians with assessments, ordering lab tests, prescription renewals, providing health teaching and offering suggestions on healthy lifestyle improvements including healthy eating, active living, weight control and smoking cessation.

The physicians of Bonnyville and Aspen Regional Health are working together to provide the best health care for people with chronic diseases in the Bonnyville area.
